Get Referred or Admitted to a Program

Getting referred

Referrals must be made by a physician. Your doctor should fax a referral to the Intake Office, fax # 604.730.7904. For information on how to make a referral, and/or for a referral form, call 604.737.6291.

The Intake Office hours of operation are Monday to Friday, 8:00 am to 3:30 pm.

• Inpatient referrals (ABI/AYA/NMS/SCI Programs) : Referral Form
• Outpatient referrals (ABI/AYA/NMS/SCI Programs) : Referral Form
• Inpatient Arthritis referrals : Referral Form

Referral must be accompanied by:

• Recent medical history/consults

For Inpatients:

• Recent diagnostic test results
